The purpose of this sheet is to provide physicians in the city of Beverly with contact numbers to fulfill their reporting duties in a suspected emergency.

All reportable diseases are reportable first to the local board of health, except tuberculosis, HIV infection, AIDS and sexually transmitted diseases.
