The 13th Annual Public Health Leadership Forum, Social Determinants of Health: Improving Population Health through Prevention Based Care will examine what can be done by policy makers and the medical community to address the impact of physical, economic, environmental and social inequities on health outcomes how the medical community can work in partnership with public health to shift the paradigm from treating disease to improving population health through prevention based care that recognizes social determinants of health when caring for patients.
 
Leaders in the field will explore the importance of shaping a health care system that recognizes and responds to the effects of social determinants in order to improve population health and health equity.
 
The forum will run from 1:00 pm - 5:00 pm.

Activity Objectives:  
After participating in this activity, you should be able to:
  • Describe the impact of economic, physical and social conditions on the  health of individuals and communities
  • Examine how unequal access to a safe and secure environment impacts health and access to care
  • Develop appropriate skills and attitudes to address all populations
  • Contribute to reduction of health disparities by providing  culturally competent care
  • Employ strategies that reduce stigma and discrimination
