Because of the bleeding risk caused by antiplatelet therapy, aspirin should be prescribed for primary prevention only in patients for whom the benefits of therapy outweigh its harms (e.g., 50-year-old with metabolic syndrome and multiple cardiovascular risk factors with low risk for bleeding).  Aspirin is generally not recommended to prevent stroke in women less than 55 years old.  Insufficient evidence exists for recommendations in men and women aged 80 years and older.
