A 71-year-old woman is seen in the emergency department with back pain and bruising after having been pushed into a wall by her husband.  She tells the emergency department nurse that she has been physically assaulted by her husband on several prior occasions but has not required medical attention before today.  X-rays are negative for fracture. She declines information about domestic violence services available in the community, insisting that her husband is "a good man," is remorseful, and that he has promised never to hit her again. The patient is offered referral to a battered women's shelter but insists on returning home.
